Case Manager

The Case Manager will work within the Youth Support Services program and ensure that all identified targets are met in line with the specified service agreement and outcomes of the program. 

The Case Manager is responsible for the day-to-day delivery of services to the specified target group (young people aged 8-21 yrs of age) and ensuring that these young people receive targeted, person-centred intervention, using a flexible and holistic service delivery model.

The position is required to provide intervention for Youth in the specified geographical catchment and deliver outcomes in the following four areas: 

  • Experience positive relationships with their family and community 

  • Engaged in education, training and/or employment 

  • Lead healthy and violence free lives 

  • Have safe and stable places to live. 

The role’s aim is to work towards improving the lives of vulnerable youth in the area and engage young people who are most at risk in the local community.
